Stanford type-2 diabetes self-management program at Regency of Evergreen Valley
The Stanford Patient Education Research Center invites all adults who have been diagnosed with type-2 diabetes to take part in a diabetes self-management workshop and research study.
There is no charge to participate in the workshop. The diabetes self-management workshop is being offered at the Regency of Evergreen Valley on 4463 San Felipe Road in San Jose and will start on Saturday, April 28 and run every Saturday until June 9 from 9 a.m. to 11:30 a.m.
Without careful management, diabetes can lead to other complications such as heart disease, liver and kidney damage, and vision problems. Participants of the diabetes self-management workshop will learn how to develop their own self-management program and receive guidance on how to prevent complications through healthy eating, exercise, stress management, effective communication, and more. Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program.
Stanford’s community-based workshops are facilitated by two trained leaders, one or both of whom are peer leaders with diabetes themselves. In addition, workshops are highly participative, where mutual support and success build participants’ confidence in their ability to manage their health and maintain active and fulfilling lives. The Stanford Patient Education Research Center has received a grant from the California HealthCare Foundation for this
study.
Participants are invited to bring a family member or friend. In addition, participants will receive no-cost testing for height, weight, blood pressure, cholesterol and hemoglobin A1c. Registration is required as
space is limited.
Workshops are offered in different community settings in the San Francisco Bay Area throughout the year.
